Maybe you should look into some type of antidepressant. I am not saying your symptoms are in your head, they are not, but when you are in constant pain anyone would get depressed. And when you are depressed your pain seems worse and the worse the pain the more depressed... Obviously it will not cure your reflux but it might help you get some sleep and to not focus on your problem all day long.
On second thought maybe something like lorazepam (sp) might help. It is a muscle relaxant but also helps with anxiety (for some unknow reason). I think it is something that you need to talk to your doctor about. You need to break the cycle of not sleeping, waking up tired, feeling bad (because of reflux AND lack of sleep).
Keep trying all options you will find one that works, but work on getting some rest first and foremost.
