a twist on Dawn Phenomenon (Diabetes board)

17th March 2008
so I have been living with Type 2 for about 7 years. a couple months ago I used Byetta for a month. I saw improvement - but my morning numbers were still high like always. so about a months ago i started insulin. I am using Lantus at night and with the use of Novolog I have gooten most of my numbers to be much better. But we were still fighting my morning numbers.
like most days. I get up about 4:30 - 5AM.
I check my self and I an under 140 (today was 134). pretty good ;)
but 1 hour and 20 minutes later when I get to work and ready to have breakfast my numbers are high - 236 today. this is very common for me to keep rising after I get up.
they used to be high when I got up. my concern is that my numbers were high today (like most all days) until I checked my self at 9:45 (135). But when I checked at 830 - it was still 174. thats 2 hours after eating.
would you all say this is dawn phenomenon or am I waiting to long to eat??

It sounds like the DP to me. And it looks like Lantus just doesn't do it for you at that time of the day. You could try switching from Lantus to NPH for your long acting insulin. NPH peaks about 6 - 10 hours after being injected. So a 10pm shot would peak at 4am - 11am the next morning. This when you are having your highest readings, so it should help. You will just have to fiddle with the dose until you get it right, and you will probably have to reduce the breakfast Novolog ;).

DP is by far the hardest thing to figure out and control. I have found that my BG can continue to rise from 5AM to 8 or even 9AM. I have found the 'trick' to break it. I have to eat a certain amount of carbs to stop it. These carbs cause a single spike, then my levels start to drop. My problem is this spike can go high, like 162. If I don't cause the spike, my BG can start at 140 for 2 or 3 hours. So I take the carbs, it causes the spike, but 1/2 hour after taking the carbs, I excercise, which causes me to drop quickly to say 100. It's either 140 for 2-3 hours or 162 for 1/2 hour with a quick decrease to 100 due to excercise. I choose the latter.

I would try a little carb shot early on to see if it break the DP.
