these are the most common cases

Acute Anxiety
Secondary to petit or grand mal seizures, they'll usually keep you on this for a short time after and continue the DOC (phenobarbital)
Benzodiazepine Tapers
Acute Alcohol/Opiate Withdrawal (DT's)
Tourette's Syndrome (more moderate cases)
IBS or chronic Spasms (if Donnatal, Flexeril, Robaxin, Soma are not effective)
