Julie, taper sounds like a plan but I would also talk to the Dr about a secondary or fall back plan if the taper gets you into a holding or its to painful or you can't stay disciplined. I know when I tapered it was steep and it hurt (mild withdrawal for 60+ days). Also if you feel the cravings are insurmountable you may want to consider Sub as that helps also curb the cravings.
Forgot to add is the reason for the edit: The Dr will determine what rates of meds to give you as your max per day may be different then mine based on a number of factors only he and you would know (body weight, other meds your taking, pre-existing conditions, etc...)
When I CT'd the Dr put me on Valium at max per day dose (40mg), he also had me take Phenergan for my stomach, ambien for sleeping, clonidine (same as catapres patch) for crawly restless legs/arms, bentyl for leg and stomach cramps and Imodium for the runs.
