   Bolus or Basal first? (Diabetes board)

I am on Amaryl, Glucophage and Starlix. It appears likely that I will need to change to insulin. I do not want to change basal and bolus at the same time. In such case how did you do it?

Did you start on a basal insulin and get that going before the bolus, or do you do the bolus and then start the basal.

I use Starlix as a bolus and I am thinking replace the Starlix with a bolus insulin but others seem to be doing the opposite. I just had a friend in the process of changing. She is super into carb counting and calculating, which I am not. She is starting the basal for the same reasons I would start a bolus, that is while you don't really know how much you need possible lows or highs. She says if you get the bolus wrong then you would be dealing with hypo or hyper. That is why I say bolus first, as if you guessed wrong initially you could correct it at the next meal. She talks about stacking insulin but I don't see a problem there. I am well controlled eating a moderate carb diet as low carbs just really mess up my digestion. She is very number conscious to the point she is almost in a "straight jacket" which I am not willing to be.

She seems to be afraid of the bolus and I am like that with the basal. It looks as though I may have to change before too much longer.

Both my friend and I are LADA. My friend is on Levemir, started 2 weeks ago. Her highs after meals is a little more than is comfortable with her, probably 140. I think that gives some room to guess low. I need more of a bolus as without the Starlix I would probably be 180 or higher.

She has Starlix which she intended to use as a bolus as I do but the first 2 doses seemed to make her sick. She did not take it anymore. I think that is unusual.

Any suggestions appreciated.
