A strong DP effect certainly suggests high insulin levels. Insulin resistant T2s tend to have a bigger problem with it than more insulin sensitive T1s. But in view of the large amount of insulin being used (91+ units a day), insulin levels are most likely high during the rest of the day too, when blood glucose levels are lower.

Insulin is the anabolic hormone, and high levels of it make losing weight difficult. The harsh reality is that there is no easy way to improve insulin sensitivity. Insulin sensitizing drugs (Actos, Avandia :() may help, but getting exercise, reducing carbs and losing fat/building muscle are the only tactics that definitely work.
